A Study of 26 cases of uterine rupture


Abstract
The gravid uterine rupture is one of the most serious and emergent problems in obstetric field. The Author observed 26 cases of the gravid uterine rupture treated duirng 7 years from October, 1969 to September, 1976.
The results of 26 case of uterine rupture were summarized as follows:
1. The incidence was 1:233 deliveries.
2. The age encompasses was from .25 to 39 years of age and the mean age is 30.1 years old.
3. Regarding the parity, para I and II was more frequent (7 case) and other cases were multipara except 4 case of primipara.
4. The gestational weeks was from 28 to 44 weeks. The most cases were ruptured at full term.
5. Regarding the causes of rupture, traumatic ruptures were observed in 10 cases. rupture of previous cesarean section in 9 cases and spontaneous rupture in 7 cases.
6. The most frequent symptoms and signs were lower abdominal pain, inaudible fetal heart sounds and shock.
7. The interval period from onset of labor to rupture were ranged from 2 hours to 5 days, the mean was 7.9 hours.
8. The site of rupture involved with lower uterine segment in 18 cases and the corpus in 8 cases.
9. In the management, subtotal hysterectomy was performed in 15 cases(57.6%), simple closure 7 caEes(26.9%) and total hysterectomy 4 cases (15.5).
10. No maternal death was encountered and fetal death rate was 88.9%.
11. The leading cause of traumatic uterine rupture was abuse of the oxytocin accounting for 50% of and it resulted in mostly by general practitioners man or midwives.
12¢¥ The occurence of rupture of previous uterine scar were obviously more often (7 out of 9) in
classical cesarean section scars rather than in the lower cervical scars.
13 The common type of uterine rupture was the complete rupture of the uterus(64%) and it has been found mostly in the lower segment.
